Serving health care providers who serve seniors in the Inland Empire and Southern California

The Inland Association for the Continuity of Care (IACC) has been in existence for the past 27 years. Originally started with a group of Social Workers and Nurses needing an avenue for networking and supporting one another in case management roles, the organization has strived for the past 27 years to support the health care community and it’s participants through education at our monthly meetings and through an annual Spring Conference.

At our last conference we celebrated 27 years of this annual event that recognizes all those who have made this association possible. Without that group we would not be here today, especially with current trends of networking organizations dissipating or struggling for attendance.

Ultimately, we all recognize that our aim is to provide quality continuity of care to the patients and families who depend on those who are in the health care field. We do our part in that care cycle through our six core compentencies outlined nearby -- all directed at helping you deliver that quality continuity of care.

If you are a professional health care provider, then this website is for you. Our goal is to consistently provide you with the latest and most current information, legislative developments, best practices, advisory services, and simple awareness about changes and trends in your field of work.



Education

Providing for the educational needs and professional growth of IACC members

Advocacy

A patient/client advocate effecting changes in the health care delivery system

Advisory

Providing informational and advisory services to members, health care providers, governmental agencies, consumer groups and the community

Competency

Promoting professional competence and expertise in the assessment, planning, intervention, and evaluation process

Awareness

Promoting the awareness of the right to access health services as it relates to continuity of care

Communication

Promoting and enhancing communication among those involved in the discharge planning, case management, and continuity of care processes

The 28th annual IACC Spring Conference came off without a hitch, as usual, and provided a chance for everyone to renew old friendships as well as gather some needed CEU credits. If you missed this year’s event, then you missed a lot of fun. Here’s hoping that we see you in 2013.
