About the IACC
The purpose and philosophy of the Association will
include:
Promoting the concept that continuity of care is an essential
component of the health care delivery
system and that this care should be delivered in the most
cost effective manner;
Promoting the concept that every patient has the right to
quality, coordinated health care beginning
prior to and upon admission to a health care facility,
including, but not limited to, discharge planning,
patient and/or family involvement in the discharge planning
process;
