About IACC

Since its founding, IACC has promoted the concept of Continuity of Care as an essential component of the health care delivery system. The handoff from hospital to Skilled Nursing Facility or Residential Care Facility for the Elderly with possible need for additional coordination of care from home health, occupational therapy, physical therapy or speech therapy allows the patient/resident to progress to home care, or as sometimes needed, palliative care or hospice care.  

Care provided in a home-like setting can have a positive and long-lasting impact on the comfort and recovery rate of the patient. We feel that this continuity of care is critical and should be delivered in the most effective and cost-efficient manner possible. This is to benefit not only the patient/resident but the immediate family or support group for that patient/resident as well.


To that end, our aim has always been to bring our multiple disciplinary providers together under the umbrella of IACC to allow for the networking and educational needs involved in the process of providing continuity of care for their respective patients/residents.  We continually strive to always offer IACC as a source for all National and State Legislation updates and Best Practices that can be of use to our members who are the ultimate care providers.

We believe and support the philosophy of continuity of care as an interdisciplinary health care approach, centered on the patient/resident and the family/responsible party. We stress the need to ensure that the preventative, therapeutic, rehabilitative, psychosocial, and medical needs are always uppermost in the minds of caregivers. These needs, as well as the care needs in the community are emphasized during the delivery of care process and discharge planning.

It is to that end that we believe the professionals involved should have access to an educational and supportive network. The Inland Association for Continuity of Care (IACC) was born from this belief 27 years ago, and it remains at the compass points of our existence today.
