for deserving students participating in their continued education in the health care

Open to anyone who is licensed or a professional who is actively involved in or responsible
for continuity of care processes, who is currently employed in the healt care field,
and who agrees with the philosophy and objectives of IACC. General members may attend
all monthly meetings and is eligible to hold Board positions.
Available to any representative of a firm, partnership, corporation, or other entity
which has a mutual interest in continuity of care through the delivery of services
and products, or to any person who wishes to contribute to the on-going efforts of
the Association and who agrees with the purpose and philosophy of the Association,
but who does not qualify as a General Member. Associate members are not eligible
to attend monthly meetings and my not serve on the Board, committees, or subcommittees.

Individual Membership: For the individual health care provider. $40 annual membership

Corp Membership (3-4) $115
Corporate Membership - 3 to 4 individuals: $115 annual membership fee will cover
up to 4 individuals. Please remember that memberships are NOT transferable. Click

Corp Membership (5-7) $195
Corporate Membership - 5 to 7 individuals: $195 annual membership fee will cover
up to 7 individuals. Please remember that memberships are NOT transferable. Click

Individual memberships are $40 each, however, our corporate rates will save money
when there are multiple individuals to join IACC. If you have more than seven members
who wish to join IACC, please call for a quote.
Within the corporate rate structure, each individual has to join as either a General
or Associate member. Definitions are just below. Please note that memberships cannot
be transferred.

Applications for membership will be processed at the next board meeting, and notification
of your acceptance or denial will be sent to you promptly. If for some reason your
application is not accepted, any money paid will be refunded in full.
